Fees
A general daily rate per employee applies to CCKL personnel assigned to an audit day. An employee may be: a team leader, professional specialist or official secretary. An Official Secretary is assigned to every audit team to support the team leader.
Accreditation registration and system assessment Candidate laboratories receive all relevant application documentation and other CCKL publications (included in the registration fees). These fees also cover the cost of preparation, system assessment and reporting. The registration valid for two years.
On-site assessment examination Actual days spent by CCKL personnel on the on-site assessment visit will be charged retrospectively. The laboratory will be notified in advance of the number of employee-days the examination is expected to take. There is no separate charge for preparation and reporting of the audit. The assessment charges for the initial examination are treated as the contribution for the first year, and must be paid after the assessment visit.
Annual contribution The annual contribution applies from the date of the on-site assessment examination, and is determined by the size of the audit team. The amount of the contribution is fixed for four years. This is settled every calendar year in January. The contribution amount is indexed annually based on inflation. Should accreditation be terminated in the course of a year, no restitution of the contribution shall take place.
Reassessments and Surveillance on-site assessments Reassessments and regular Surveillance on-site assessments are covered by the contribution. The amount of the contribution may be adjusted in the fifth year, based on the new number of employees required for the reassessment. This means that an additional amount may be payable retrospectively, based on this recalculation.
Validity of accreditation The accreditation applies for five years, except when a shorter period has been agreed with the institution. The contribution is based on reassessment taking place once every four years and Surveillance on-site assessment taking place once every four years (so every two years an on-site visit).
Costs in the event of a cancelled assessment Cancellation fees will be chargedt if an institution cancels an assessment that CCKL has previously confirmed to the institution in writing.
Framed certificate of accreditation Following accreditation, one framed certificate of accreditation per accredited laboratory will be handed over at the first assessment. Additional framed certificates are available at cost price.
Addenda to certificate of accreditation Following accreditation, one laminated copy in both Dutch and English of the certificate of accreditation, including addenda, will be provided for the purpose of customers, submitters, trials and research. Additional copies are available at a charge.
